Practice by Notification Form

PURSUANT TO 5 U.S.C. 552a, THE KANSAS BOARD OF ACCOUNTANCY ADVISES YOU THAT SOCIAL SECURITY NUMBERS PROVIDED TO THE BOARD PURSUANT TO K.S.A. 74-148 AND 74-139 MAY BE PROVIDED TO THE KANSAS DEPARTMENT OF REVENUE, UPON REQUEST, OR MAY BE USED FOR CHILD SUPPORT ENFORCEMENT PURPOSES.

Note: This form applies ONLY to the following applicants: 
1. Applicant does not have a principal place of business in Kansas; 
2. Applicant does not have a Kansas CPA certificate;
3. Applicant has an active CPA license/permit issued by a state other than the State of Kansas; and 
4. Applicant's license/permit is from a state whose qualifications for CPA licensure are substantially equivalent to the education, examination, and experience requirements of the UAA; or if the State's qualifications are not substantially equivalent, the Applicant's qualifications are substantially equivalent. 

THE AUTHORITY TO PRACTICE BY NOTIFICATION IS VALID FOR 2 YEARS FROM ITS ISSUE DATE AND MUST BE RENEWED THEREAFTER IN ORDER TO CONTINUE PRACTICING IN THIS STATE.  THIS FORM MAY ALSO BE USED FOR RENEWAL OF THE AUTHORITY TO PRACTICE BY NOTIFICATION.

Certificate Forms

If you are applying for a Certificate by Reciprocity using NASBA's Qualification Service, visit the NASBA web site and go to the "Credential Net" link for the appropriate forms.
